Instructions for Making Contributions to the Rotary Foundation

- Checks should be made out to “The Rotary Foundation“. Do not put anything on the MEMO line (such as Polio).
- Use DACDB report #2 — Rotary Foundation Transmittal. By using this report you will not have to look up member numbers etc. Put in the amounts of the contributions, run the report, mark the purpose of the gifts as Annual Fundat the bottom,.
- Note that this report only prepares the transmittal — it does not electronically interface with TRF records.
- IF THE CONTRIBUTIONS ARE FOR THE FOUNDATION ANNUAL FUND
- mail the DACDB Transmittal Report #2 and the checks to the Foundation address shown on the Transmittal Report.
- IF THE CONTRIBUTIONS ARE FOR POLIO,
- use DACDB Transmittal Report #2, mark ANNUAL FUND at bottom, make checks payable to “Rotary Foundation” and do not list “polio” on the check or memo.
- Mail the Transmittal Report and the check(s) to Bob Browning, D6580 Foundation Chair along with a separate note that it is for POLIO.
- Sherry will give the club and the individuals Annual Fund credit and allocate DDF to fulfill the donor’s wishes to support the Polio campaign.
- IF YOU WANT TO USE THE DISTRICT MATCH, use DACDB Transmittal Report #2, mark ANNUAL FUND at bottom, mail the report and checks to Sherry Nielsen, along with note of who wants the match credits. Sherry will attach the appropriate paperwork and forward to TRF.
